Auto Claim Information Needed

  • Policy Number
  • Name, Address, Phone number, Date of Birth
  • Date, time & location of accident/ loss
  • Weather/ road conditions
  • Vehicle information (year, make, model, Licence plate)
  • Name of driver, passengers, registered owner of vehicle
  • Any injuries
  • Any others involved (Third party insurance information, vehicle description, driver & passenger names, any injuries
  • Witness information
  • Identify if you have any urgent needs ie: rental car required, etc.
  • If the accident reported to police, emergency workers, are there any charges laid
  • Accidents must be reported to Police if damages to vehicle exceeds $1000, if there are any injuries sustained by anyone involved
  • Or instructed by your claims advisor
  • Description of loss so fault can be determined
  • Where vehicle is located and if it is drivable

Depending on situation additional information may be required or forms may be needed to be completed.
